SpiriTrust Lutheran is hiring a FT Day RN Home Health and Hospice
80 Hours per bi-weekly pay. 8:00am-4:30pm.

General Responsibilities
Responsible for providing skilled nursing services and management of client cases as assigned; coordinating work within the department, as well as with other departments; complying with operating policies and procedures and regulatory requirements; communicating with appropriate personnel; responding to inquiries or requests for information; maintaining appropriate records and providing assigned reports.
Basic Qualifications
Education/Training: Graduate of a school of nursing with current license(s) to practice as a professional nurse in the state(s) served by SpiriTrust Lutheran Home Care & Hospice; current CPR certification within three (3) months of employment.
Skill(s): Proficient reading, writing and grammatical skills; proficient in interpersonal relations and communication skills, including pleasant telephone manner and speaking voice; ability to work independently and to establish and maintain effective working relationships; working knowledge of computer operations and ability to learn specialized industry applications; demonstrates a desire and ability to work with terminally ill clients and with bereaved individuals and/or families; knowledge of currently acceptable practice; valid driver’s license and dependable means of transportation.
Experience: One (1) year of recent clinical experience in licensed or certified discipline normally required.
Essential Duties

Assesses and evaluates the client’s need for Skilled nursing services.

Travels to client’s location in various weather conditions.
-
Gains access to the client by available means which may include but is not limited to steps; uneven surfaces; wet, snow covered or icy surfaces; narrow entrances; etc.

Assesses visual, olfactory, tactile and auditory signs and skilled nursing needs of the client.
-
Communicates with client to identify and assess symptoms and reactions of client’s conditionand reports same.

Identifies and prioritizes nursing diagnosis and determines realistic goals in cooperation with client/ family and/or attending physician.
-
Observes and reports the social and emotional factors relating to the client’s condition, need for care, response to treatment and adjustment to care.

Plans, organizes and implements nursing interventions according to prescribed care, initially and at each recertification period.

Performs procedures and techniques correctly and safely with the resources available at the client’s location.

Executes medical regimen.

Moves or assists with the movement of adult and pediatric clients with or without assistance as available.

Improvises materials and resources to meet the need of client/family/significant other.

Teaches client/family/significant other and obtains return demonstration and verbal understanding.

Evaluates safety concerns and implements preventative measures as appropriate.

Participates in establishment and evaluation of nursing care plan.

Teaches, supervises and assigned Home Health Aides/Hospice Aides and LPNs measures to implement the nursing care plan.

Takes an active role in discharge planning starting with initial visit.

Coordinates care in cooperation with the Clinical Supervisor as follows:
-
Establishes client eligibility for admission and obtains appropriate consents. Ensures client continues to meet eligibility criteria throughout course of care.

Coordinates intra/inter agency services and other community resources.
-
Establishes frequency of nursing visits.
-
Assures the accuracy and completion of client chart and physician’s order in a timely manner.
-
Assures the implementation of discharge policies and procedures.
-
Is aware of and follows case management procedures.
-
Assures accurate OASIS completion for appropriate home health clients within twenty-four (24) hours of admission, recertification, resumption or discharge.

Reviews reimbursement information with Clinical Supervisor for home health/hospice clients.

Evaluates client’s appropriateness for telehealth.

Manages services to Hospice clients as follows:
-
Visits Hospice clients for the purpose of carrying out the Hospice plan of care developed by the casemanager and/or interdisciplinary Group.

Provides continuous care to clients as assigned.

Maintains appropriate documentation in accordance with Hospice Procedure.

Supports bereaved families at time of death.

Provides Bereavement Services as assigned.

Coordinates inter-disciplinary care and refers to support staff as needs identified.

Cooperates with, participates in and supports the adherence to all internal policies, procedures and practices, laws and regulatory requirements.

Works harmoniously with others.

Coordinates specific work tasks with other personnel within the department as well as with other departments in order to ensure the smooth and efficient flow of information.

Communicates with his/her supervisor, other supervisors, community relations coordinators and appropriate staff in order to integrate activities.

Responds to inquiries relating to his/her particular area, or to requests from other personnel, clients, etc. within given time frames and within established policy.

Maintains appropriate records and provides assigned reports.

Supports and advances the Lean philosophy in all endeavors.
